Student Driver Forms

Want to Get Your Driver's Permit?

You will need a School Compliance Verification Form.

Please follow these steps in order to obtain your School Compliance Verification Form:


Email Mrs. Shannon at smccoy@nhcky.org to request a form.

  • Include the student’s full legal name.
  • Mrs. Shannon will have the form available at the front office the next school day for pickup. 
  • Make your appointment with the DMV. The form is good for 60 days after it is issued.
